Include date(s), time(s), and location(s) of incident

Describe the people involved. If you can, include badge or other official ID numbers, if not then include approx. age, height, build, hair length, colour and style, eye colour, ethnic background, clothes and shoes worn, & any distinguishing features.

Describe any vehicles involved inc (if known) registration (or part registration plate) make, model, colour, accessories and any distinguishing features. If no vehicles were involved, please state "None".
